How can I pre-register a patient?
1. Choose between "Dependent Registration" and "Self Registration"
2. Insert patient or guardian information. Your form may look different than what is pictured below based on your choice in step one.
3. Choose how to be contacted (e-mail and/or SMS).
4. Read and accept the terms of laboratory testing.
5. Users that chose "Self Registration" can hit the "Submit" button and are done. For those that chose "Dependent Registration, there are just a few additional steps. Next you will need to select "Add" for each dependent.
6. Complete the "Dependent Information" form.
7. After the form is completed, hit "Add".
8. Hit the "Submit" button.
